Context poisoning

When outdated attempts, failed paths, and conflicting instructions stay in the window and start steering the model in the wrong direction.

The model cannot reliably separate old intent from current intent, so mistakes compound over time unless context is reset or cleaned aggressively. This is exactly what good context engineering aims to prevent.
